East Brunswick School of the Arts Annual International Program Permission Form & Parent/Student Contract for Rome 2024

 

General Expectations and Guidelines

  1. Middlesex County Magnet Schools and/or the MCMS East Brunswick campus administration reserves the right to exclude students from participation in this trip due to: poor academic standing, failure to fulfill financial obligations, disciplinary infractions, or habitual school tardiness and attendance. Participation on this field trip will be at the absolute discretion of the MCMS Board of Education and/or the East Brunswick administration who have the ultimate authority to make such determinations regardless of money paid for the trip. No refunds will be given in cases where violations, abuses, or inappropriateness, and/or failure to follow the rules and regulations of Middlesex County Magnet Schools, and/or local, state, or federal laws have occurred. Any such determinations will be considered on a case by case basis and such violations and/or infractions to Middlesex County Magnet Schools policies and procedures and/or MCMS School of the Arts International Program’s code of conduct and/or violation of any local or federal law will be grounds for exclusion from the trip. 

 

  1. The tour leaders and chaperones expect the highest level of cooperation from everyone attending the trip. Students must follow any instructions given by all adults, even if the adult is not an official school chaperone. Students are to conduct themselves at all times with maturity and dignity. Regardless of any foreign country the East Brunswick School of the Arts Annual International Program travels to or may visit, the rules and regulations of Middlesex County Magnet Schools and the laws of The United States of America shall apply at all times. During the trip any student found with or suspected of drug/alcohol possession, use, or distribution or in violation of either Middlesex County Magnet Schools rules and regulations, local law, and/or the laws of the United States of America if not answering to civil authorities will be sent home immediately accompanied by an adult chaperone at the complete and total expense of the parents/guardian of the student in violation. Parents/guardian agree to pay all monetary costs associated in such an event including airfare for both the student and chaperone, all transportation costs, taxes, fees, and any and all additional expenses associated with the immediate return to the United States of America of the student in violation. Upon return, the violating student shall also face appropriate disciplinary action by the MCMS East Brunswick administration.

 

  1. The School of the Arts Annual International Program is open to any East Brunswick Campus student, faculty member, faculty affiliate chaperones, administrator, alumni, office staff or current student family members may all participate in the international program. Please keep in mind that there are a limited amount of spaces and MCMS East Brunswick administration must approve any student who signs up and/or places a deposit. If a student is unable to go due to MCMS East Brunswick administrative denial or lack of space any monies paid (deposit) will be refunded by Celestial Voyagers Inc.   

 

  1. No student at any time may break away or be alone during the trip. Students must remain in established cluster groups of no less than three persons at all times during the duration of the trip. Student participants may be given free time to shop or sightsee but must always remain in their designated groups of three persons or more at all times and in the designated areas outlined by tour leaders or chaperones. No student may leave, venture away, or deviate from the established itinerary without direct tour leader permission. Chaperones’ orders concerning meeting times are to be followed strictly and no student is permitted out of their hotel rooms after final nightly room checks. 

 

  1. Students wishing to attend Rome 2024 must be highly motivated, interested in art, culture and be willing and able to do a great deal of walking on a daily basis. Students for Rome 2024 must also go through a vetting process by administration, guidance and see Mr. Tiedemann and Ms. Mack for an in-person interview to determine eligibility which shall be at the discretion of the school district and/or chaperones of the MCMS School of the Arts International Program.

 

  1. Tips are not included in the cost of the trip and it is required that all participants tip appropriately for all guides. It shall be mandatory to tip our daily (single event) or specialty guides between 3 to 5 Euros for their services (this also includes our bus drivers). Our 24/7 local guide shall receive a tip between 20 to 30 Euros for the entire week which will be collected by the chaperones on or about the last day of the trip. It is highly recommended that each participant calculate and set aside 5 to 10 Euros per day for tipping purposes. 

 

  1. Breakfast and Dinner are included in the cost of the trip however lunch is not. Provided meals will accommodate dietary requirements and/or restrictions and there will always be a vegetarian/vegan options available as long as each participant notifies Celestial Voyagers Inc. of their specific dietary needs. NO STUDENT WILL BE PERMITTED TO LEAVE THE HOTEL TO OBTAIN FOOD FOR PROVIODED MEALS (BREAKFAST AND DINNER). Students must take part in the communal group meal times and eat the options provided by our hotel and/or scheduled restaurant establishments. It is the responsibility of all participants to notify Celestial Voyagers Inc. of any and all food allergies, dietary requirements, and/or requirements.  

 

  1. I agree to pay the cost of the field trip to be paid in installments. This includes round trip airfare, hotels, breakfast and dinner daily, personal guides, admission to all events/attractions/museums, and airport transportation. Daily lunch, tour guide tips, single room supplement, cancel anytime insurance, passport & visa fees. Personal expenses, and spending money are not included and are the responsibility of each participant. Please make your initial deposit payment online to Celestial Voyages Inc. NO PAYMENTS ARE ACCEPTED BY MCMS! All remaining subsequent payment installments to be paid by phone via credit/debit card or a mailed check or money order payable to Celestial Voyagers Inc. Please indicate (MCMS Spain Payment #) in the memo portion of the check. NO CASH WILL BE ACCEPTTED! ONLY THE FIRST PAYMENT IS TO BE MADE ONLINE AT WWW.CELESTIALVOYAGERS.COM. ALL OTHER PAYMENTS ARE TO BE MADE TO CELESTIAL VOYAGERS BY MAIL OR BY PHONE. MCMS WILL NOT ACCEPT ANY PAYMENTS! PLEASE DO NOT MAKE OR SEND ANY PAYMENTS TO MCMS! The East Brunswick School of the Arts Annual International Program and the School of the Arts PTO may conduct fundraising efforts and/or make attempts to obtain sponsorship to offset the cost of the trip as much as possible for student participants only. It will be a mandatory requirement that if any fundraising events are scheduled that all students participating in the East Brunswick School of the Arts Annual International Program must participate in any/all fundraising efforts for their excursion, attend all established meetings, meet reasonable physical requirements, have and maintain a 3.0 GPA all year, and write a 500-word essay on why they wish to attend the international trip to Rome, and upload a copy of their passport to both Celestial Voyagers and to the ROME 2024 DOCUMENT UPLOADS GOOGLE CLASSROOM PAGE

 

  1. All permission forms, parent/student contracts, medical forms will be due as soon as possible or no later than October 31, 2022. This trip will be booked on the School of the Arts International Program’s point system pending approval of the Middlesex County Magnet Schools and/or the MCMS East Brunswick campus administration. We have a limited amount of spots available (40 Spots). No student will be permitted to attend without all necessary parental permission forms, medical information, 500-word essay, monthly meeting attendance, passport copies, and parent/student contracts signed as well as all fees paid on the required due dates. The Celestial Voyagers payment schedule is as follows:

10.. Celestial Voyagers Inc. Refund Policy: Once someone registers and makes their initial deposit(s) and/or subsequent additional payments, there will be absolutely no refunds given. (with only two exceptions):

  1. A serious illness or injury. Any serious illness or injury must be verified by a licensed physicians note stating that the individual(s) cannot travel due to the nature of the illness or injury. Any anxiety or mental illness issues must also be verified by a mental health professional and disclosed to the MCMS tour leaders in writing no later than 15 days after the initial deposit is made in addition to being disclosed on the Spain 2023 medical portion of this permission form and also the separate medical form. Failure to do so will not be cause for any refund.
  2. A death in the immediate family only! (Parent, sibling, or grandparent) shall be the only family members which constitute “immediate family”! Extended family and/or friends do not apply! This situation must also be verified by written documentation from a qualified professional such as: (death certificate from a doctor and/or official letter from a funeral director) no later than 15 days after the date of death. Failure to do so will not be cause for any refund.
  1. All students attending must write a 500-word essay (Why I want to go to Rome, Italy) which will be due to the MCMS International Program’s Google Classroom Page by Thursday, October 31, 2023.   1. All students going to Rome 2024 must attend the monthly Rome 2024 meetings in Mr. Tiedemann’s classroom with Ms. Mack S-117 during their designated lunch period. Any student who misses three meetings or more will be automatically dropped and excluded from attending the trip with no refunds given. Dates for the monthly Spain meetings will be posted on Mr. Tiedemann’s Multimedia shop website under the menu tab ROME 2024.

I give permission for my child/children and/or individual for whom I am a legal guardian to participate in the East Brunswick School of the Arts Annual International Program to Rome 2024. I recognize the extent of the reasonable and prudent responsibilities as well as those of my son/daughter in abiding by set school regulations and policy. We (parent/guardian & student) have read, understood, and abide by the guidelines and expectations listed in this permission form/contract and on all the other forms which pertain to this trip. We (parent/guardian & student) have informed the tour leaders and school officials of any and all medical, dietary, and/or medication needs and they are authorized to dispense approved medicines according to the parent/guardian’s and/or physician’s instructions. In the event of an emergency, all doctors require parental consent before administering treatment. In the event that I cannot be reached in an emergency, I hereby give my consent to have my child/children and/or individual for whom I am a legal guardian treated by a physician and/or hospital. I certify that my child/children and/or individual for whom I am a legal guardian is/are in good physical condition, or that I have/will listed any and all specific medical conditions on the official MCMS medical form digitally via the ROME 2024 DOCUMENT UPLOADS GOOGLE CLASSROOM PAGE. As the parents/guardian by clicking on this form and making a deposit I consent to and agree with all of the information outlined above and within it and all other accompanying forms. I understand that I shall be fully responsible for any and all expenses which may be incurred in cases of violation of terms and or this contract. I also understand and agree this is a legal and binding contract and will provide all the necessary and truthful information requested by Middlesex County Magnet Schools, the East Brunswick School of the Arts Annual International Program leaders and chaperones, and Celestial Voyagers Inc. I understand that although the students will be supervised by program leaders and chaperones, I do assume the risks in my student’s participation in the event. I acknowledge that I will not seek to have the School District, its tour leaders, chaperones, or any participant held liable in the event that any accident, injury, loss of property or any other circumstances or incidents which may occur during or as a result of my son’s/daughter’s participation in the field trip. This release of liability includes accident, injury, loss, or damages to the student, as well as, to other individuals or property which may result from the student’s participation in the event. I hereby release and agree to hold harmless the School District, its officials, agents and employees, from any claims arising out of my son’s/daughter/child/children and/or individual for whom I am a legal guardian participation in the event(s). I also read, understand, abide by the cancellation policy set forth by Celestial Voyagers and accept the terms regarding refunds or reimbursement due to cancelations, events, or acts of God beyond our control including but not limited to pandemics, wars, civil unrest, natural disasters, infrastructure and/or technology breakdowns and/or failures, or any other circumstances or events beyond the control of Middlesex County Magnet Schools, the East Brunswick School of the Arts Annual International Program leaders and chaperones, and Celestial Voyagers Inc.

 

I have read and understand and accept all of the statements in this permission

form/parental contract and accept full responsibility as described. By clicking on this form and/or making a deposit payment I acknowledge that I agree and have signed and accepted all the terms of this and all other official trip forms.

 

UPON RETURN TO MCMS EAST BRUNSWICK CAMPUS RETURN TIME IS SUBJECT TO CHANGE BASED ON FLIGHT DELAYS, TRAFFIC, AND/OR WEATHER CONSITIONS. IT IS THE RESPONSIBILITY OF THE STUDENT AND PARENT/GUARDIAN TO MAKE ARANGEMENTS TO GET HOME FROM THE EAST BRUNSWICK CAMPUS. ALL STUDENTS SHOULD CARRY A CELL PHONE AND KEEP THEIR PARENT/GUARDIAN UPDATED ESPICALLY WHEN WE ARE ARRIVING IN THE UNITED STATES OF AMERICA PRIOR TO LANDING GIVING PARENTS AMPLE TIME TO BE THERE TO PICK UP THIEIR CHILDREN.
